When A Surgical Face-Lift Is The Best Option
Many patients come to my office believing a facelift is their only solution. In reality, more than half ultimately achieve the improvement they want through non-surgical treatments and are very happy with their results.
That said, not every face can be corrected without surgery.
When Non-Surgical Treatments Aren’t Enough
In patients with significant skin looseness, non-surgical approaches may improve skin quality but cannot adequately tighten excess skin. In these cases, surgical removal of skin is necessary to achieve a balanced, natural result.
Whenever surgery is appropriate, my priority is to:
• Use the least invasive approach possible
• Minimize incisions and visible scarring
• Preserve a natural, non–operated look
ENDOSCOPIC FACE-LIFTS
Some patients with moderate laxity are excellent candidates for an endoscopic facelift. This technique uses very small incisions hidden within the hairline, allowing repositioning of deeper facial tissues with minimal scarring. When appropriate, it may be combined with skin resurfacing or other supportive treatments to enhance skin texture and overall refinement.
When a More Comprehensive Facelift Is Needed
Other patients have more severe skin looseness combined with multiple aging changes. In these cases, a more complete facelift may be required.
There are several surgical approaches, including:
• Subcutaneous facelift (just beneath the skin)
• SMAS facelift (lifting the deeper support layer of the face)
• Deep-plane facelift
• Sub-periosteal facelift
Each technique has specific indications and must be carefully matched to the patient’s anatomy, skin quality, and goals.
While deeper techniques can offer powerful correction, they typically involve:
• Longer operating times
• Larger incisions
• More swelling and recovery
• Greater risk of visible scarring or changes to the hairline or ear position
If not properly selected or executed, overly aggressive lifting can lead to the classic “wind-blown” or over-operated appearance—something I work deliberately to avoid.
A Personalized, Balanced Decision
There is no single “best” facelift. The right solution is determined by:
• Your degree of skin laxity
• Skin quality and facial structure
• Tolerance for downtime
• Long-term goals for how you want to look
